Your path to wellness with Reconnect

At Reconnect Physiotherapy, we prioritise your comfort and well-being throughout your treatment journey. We’re here to listen and understand your experience, then provide a treatment plan that works for you. Here's a glimpse of what you can expect:

Prior to Your Initial Appointment:

Before your first visit, you may receive some questionnaires to complete. These valuable insights help Rachel gain a better understanding of you and how your condition is affecting your daily life.

First Appointment - Assessment and Goal Setting:

During your initial appointment, you'll undergo a comprehensive assessment. Rachel will review the assessment findings and engage in a collaborative discussion with you about your goals and the treatment plan ahead.

Collaboration and Referrals:

Rachel understands the importance of a multidisciplinary approach to healthcare. She aims to establish connections with all your treatment providers and can facilitate referrals to other appropriate specialists if needed.

For ACC Pain Management and Training for Independance Referrals:

If you've been referred to Rachel for an ACC Pain Management of Training for Independance program following your assessment, Rachel will collaborate with an extended interdisciplinary team. This team could include pain specialists, occupational therapists, psychologists, physiotherapists and dieticians. Together, they will discuss your case and design a holistic plan tailored to help you achieve your goals and aspirations.
